Note on pdf-version of the survey

We have had some reports of difficulties when filling in the offline (pdf) version of the survey. The pdf-version has been tested with different versions of Adobe Acrobat and Preview on OS X, but e.g. Adobe Reader seems to lack some of the functionality needed.
